Overview
Semax is a heptapeptide - a synthetic analogue of the adrenocorticotropic hormone (ACTH) fragment 4โ10, with an added Pro-Gly-Pro tripeptide for metabolic stability. Developed at the Institute of Molecular Genetics of the Russian Academy of Sciences in the 1980s, it has been an approved pharmaceutical in Russia since 1994. It is prescribed for stroke recovery, cognitive impairment, peptic ulcers, and optic nerve disease. Unlike most nootropics, Semax has a substantial body of human clinical data (primarily from Russian studies) and decades of clinical use in a regulated medical system.

Semax modulates BDNF (brain-derived neurotrophic factor) and NGF (nerve growth factor) expression - two proteins critical for neuronal survival, growth, and synaptic plasticity. It also affects the expression of several neurotransmitter systems including dopamine, serotonin, and acetylcholine. Its neuroprotective effects appear to involve reduction of oxidative stress, inhibition of inflammatory cytokines in neural tissue, and enhancement of cerebral blood flow. Unlike stimulants, it does not directly increase catecholamine release but rather modulates the sensitivity and expression of receptors.

Intranasal delivery
One of Semax's practical advantages is its intranasal delivery. The nasal mucosa provides direct access to the brain via the olfactory nerve pathway, bypassing the blood-brain barrier. This means faster onset (typically 5โ15 minutes), higher brain bioavailability compared to systemic administration, and no needles required. Semax nasal drops or spray are the standard pharmaceutical form in Russia. For research use outside Russia, it is typically supplied as a nasal solution.
N-Acetyl Semax and Amidate variants
Several modified versions of Semax exist. N-Acetyl Semax (NASA) has an acetyl group added to the N-terminus, which may increase stability and potency. N-Acetyl Semax Amidate (NASA Amidate) has additional modification at the C-terminus. These variants are more common in the nootropics community outside Russia. However, they have less clinical research than the original Semax formulation. Most of the published safety and efficacy data applies to the original unmodified peptide.
Semax vs traditional nootropics
Unlike stimulant-based nootropics (caffeine, modafinil, amphetamines), Semax works through neurotrophic mechanisms rather than catecholamine manipulation. This means no tolerance buildup, no crash, no cardiovascular stimulation, and no dependency risk. The trade-off is that effects are more subtle and cumulative - users typically report gradual improvement in clarity, focus, and verbal fluency over several days rather than an immediate cognitive boost. This makes it more suited to sustained cognitive work than acute performance demands.
